    Description

    Esomac® Granules is manufactured by Cipla. The active ingredient of this treatment is Esomeprazole, proton pump inhibitor. It works by reducing the acidity in the stomach.

    Indications:

    - gastroesophageal reflux disease (GERD) characterized by heartburn (acid indigestion), chest pain, dysphagia, dry cough, and acid reflux;

    - erosive esophagitis due to GERD;

    - peptic ulcers due to Helicobacter pylori infection and peptic ulcer disease (in combination with antibiotics);

    - gastric acid hypersecretory conditions (e.g. Zollinger-Ellison syndrome).

    Esomac® Granules are also used off-label to treat or reduce the risk of NSAID-associated gastric ulcer in patients taking non-steroidal anti-inflammatory drugs (NSAID).

    MOA (mechanism of action):

    Esomeprazole decreases gastric acid secretion by inhibiting the activity of the gastric (H+, K+)-ATPase enzyme pump. By this action, Esomeprazole blocks the final step of acid production, therefore reducing the gastric acidity and relieving the symptoms of acidity-associated disorders. Active towards both basal and stimulus-induced acid secretion. On the top of that, Esomeprazole possesses strong activity against Helicobacter pylori (H. Pylori) bacteria.

    Dosage

    Esomac® Granules are for oral use only.

    Esomac® Granules should be taken at least 1 hour before meals.

    The exact dosage as well as course duration should be indicated by your doctor.

    Esomac® Granules should be mixed with water before taking. You need 15 ml of water (3 teaspoons) for each sachet.

    You should take Esomac® Granules in no longer than 30 minutes after the suspension was prepared.

    Do not give Esomac® Granules to newborns under 1 month of age.

    Gastroesophageal reflux disease:

    The dose for adults and children over 12 years is 20 mg once daily for 4 weeks.

    The dose for children aged from 1 to 11 years is 10 mg once daily for 8 weeks.

    Erosive esophagitis:

    The dose for adults and children over 12 years varies from 20 to 40 mg once daily for 4-8 weeks.

    The dose for children aged from 1 to 11 years weighing under 20 kg is 10 mg once daily for 8 weeks.

    The dose for children aged from 1 to 11 years weighing 20 kg and more varies from 10 to 20 mg daily for 8 weeks.

    The dose for children aged from 1 month to 11 months weighing 3-5 kg is 2,5 mg daily for 6 weeks.

    The dose for children aged from 1 month to 11 months weighing 5-7 kg is 5 mg daily for 6 weeks.

    The dose for children aged from 1 month to 11 months weighing 7,5-12 kg is 10 mg daily for 6 weeks.

    Peptic ulcers in patients suffering from H. pylori infection and peptic ulcer disease:

    The average adult dose varies from 20 to 40 mg daily for 6 months.

    Gastric acid hypersecretion (e.g. Zollinger-Ellison syndrome):

    The average adult dose is 40 mg twice daily.

    NSAID-associated gastric ulcer:

    The average adult dose varies from 20 to 40 mg daily for 8 weeks.

    How to take Esomac® Granules:

    1. Take an empty clean glass and fill it with 15 ml (3 teaspoons) of water for each sachet. Meaning, you will need 30 ml (6 teaspoons) for two sachets, 45 ml (9 teaspoons) for three, etc.

    2. Carefully open the sachet and empty its contents into the glass with water. Use as many sachets at once as prescribed by your doctor.

    3. Stir the granules in the water until well mixed.

    4. Leave the mixture for a few minutes until it has thickened.

    5. After the suspension has become thicker, stir again lightly and drink immediately.

    6. If anything remains in the glass, add some water and drink the residue.

    Missing a dose

    If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose.

    Overdose

    If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature between 10 and 25 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Side effects

    The most common side effects associated with the use of Esomac® Granules are:

    • atrophic gastritis
    • acute interstitial nephritis
    • cyanocobalamin (vitamin B-12) deficiency
    • osteoporosis-related bone fracture

    Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

    Contraindications

    Do not take Esomac® Granules if you are allergic to Esomeprazole or any other proton pump inhibitor (pantoprazole, lansoprazole, rabeprazole or omeprazole).

    Do not take Esomac® Granules if you are allergic to any of the other ingredients comprised in these granules.

    Do not take Esomac® Granules if you have sugars (sucrose or glucose) intolerance.

    Do not take Esomac® Granules if you have diarrhea, severe liver or kidney problems, osteoporosis, or if you are about to be blood tested for Chromogranin A.

    Do not take Esomac® Granules in combination with HIV drugs (nelfinavir, atazanavir), blood clots preventives (clopidogrel), antifungals (ketoconazole, itraconazole, voriconazole), cancer medications (erlotinib, methotrexate), muscle relaxants (diazepam), antidepressants (citalopram, imipramine, clomipramine, St. John’s wort), epilepsy drugs (phenytoin), anticoagulants (warfarin, coumarin), intermittent claudication drugs (cilostazol), indigestion treatments (cisapride), heart medications (digoxin), immunosuppressives (tacrolimus), and tuberculosis treatments (rifampicin).

    Do not give Esomac® Granules to newborns under 1 month of age.

    Esomac® Granules should be taken during pregnancy only if the potential benefits for mother justify the potential risk for fetus.

    Esomac® Granules should not be used during breastfeeding.

    Symptomatic response to Esomac® Granules therapy does not exclude the presence of gastric cancer.

    If you are going to have your neuroendocrine tumors investigated, stop taking Esomac® Granules at least 14 days before the diagnostics.
