    Description

    Tiova® Rotacaps are manufactured by Cipla and used in the management of asthma and chronic obstructive pulmonary disease (COPD). The active ingredient present in the formulation is Tiotropium, a bronchodilator belonging to the group of long-acting anticholinergics. It has an anticholinergic effect and it antagonizes the action of acetylcholine. The medication is a long-acting muscarinic antagonist which acts on muscarinic receptors on the smooth muscles of airways, opens the airways, thereby increasing the flow of air to the lungs and improving breathing. Regular long-term use of Tiova® Rotacaps helps reduce symptoms of asthma and COPD such as shortness of breath, chronic cough with sputum production, chest tightness, and wheezing, however, the medicine does not produce a sudden relief and is not useful for an acute bronchospasm.

    MOA (mechanism of action):

    Tiova® Rotacaps contain the bronchodilator Tiotropium in powder form, which is a long-acting anticholinergic that acts on specific muscarinic receptors in the bronchioles of the lungs to inhibit the interaction of acetylcholine (a neurotransmitter or chemical messenger of the nervous system) with muscarinic receptors, preventing spasm of the airway wall caused by acetylcholine. Blocking the action of acetylcholine in the airways, causes the smooth muscle in the airway walls to relax and allows the airways to dilate (widen) which increases airflow into the lungs and relieves bronchospasm (narrowing of the airways). Inhaled Tiotropium goes directly into the lungs and very little is absorbed into the blood circulation.

    Dosage

    The medication should be administered by the dry powder oral inhalation only using the Cipla Rotahaler/Revolizer. Do not swallow the rotacaps. Do not exceed the recommended dose.

    Adults and children over 12 years of age: 1 rotacap inhaled once daily.

    How to use Tiova® Rotacaps with the Cipla Revolizer device:

    1. Hold your Revolizer at the base, open it with one hand and pull back the mouthpiece.

    2. Insert the rotacap into the rotacap chamber with the transparent end facing down.

    3. Close the mouthpiece firmly. A 'click' sound will indicate a proper closing of the Revolizer.

    4. Breathe out fully, through the mouth.

    5. Place the mouthpiece between the teeth and close the lips tightly around it.

    6. Keep your head straight and breathe in through the mouth rapidly and deeply.

    7. Remove the Revolizer from your mouth and hold your breath for about 10 seconds or for as long as it is comfortable and then breathe out normally.

    8. In case some powder remains in the rotacap chamber, repeat step 4. Ensure that no powder remains in the rotacap chamber.

    9. Open the mouthpiece after every use and discard the empty rotacap.

    How to clean your Revolizer device:

    1. Open the mouthpiece of the Revolizer.

    2. If there is an empty rotacap in the chamber, remove it.

    3. Rinse the mouthpiece and rotacap chamber in clean running water.

    4. Shake well to remove excess water and leave it to air dry.

    5. Clean the Revolizer at least once a week.

    6. Never use the Revolizer when it is wet.

    Missing a dose

    If you miss a dose of Tiova® Rotacaps you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose. Do not take more than 1 dose in 24 hours.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Do not freeze the medication. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Do not use the rotacaps if you have hypokalemia or hyperglycemia.

    Do not take the medication if you have diabetes.

    Do not inhale the drug if you have thyroid problems.

    Do not take Tiova® Rotacaps if you have enlarged prostate or urination problems (due to bladder obstruction).

    Do not use the preparation if you have eye problems, such as glaucoma, as Tiova® Rotacaps can make your glaucoma worse.

    Q: How to use Tiova® Rotacaps?

    A: Tiova® Rotacapsby by Cipla should only be taken with the Rotahaler device through your mouth (oral inhalation). First, you need to take one of the capsules from the packaging and insert it into the Rotahaler. Twist the Rotahaler to pierce open the capsule. After the capsule is pierced, press the canister to get a puff and take a complete dose of the medicine by breathing in the powder by mouth two times. Remove the inhaler and hold your breath for 10 seconds. Rinse your mouth after using the inhaler to prevent dry mouth and throat irritation. Throw away any capsules that are not used right away after it is taken out of the package.

    Q: Is it safe to use Tiova® Rotacaps during pregnancy or while breastfeeding?

    A: The medication should be administered during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Tiotropium, contained in Tiova® Rotacaps, is excreted into human milk, but since many medicines are excreted into human milk, caution should be exercised if Tiotropium is administered to a nursing mother.
